Intrusive thoughts vs ability to act on them

I have had intrusive thoughts about self harm on and off for a few years now. When it comes, it scares me. I worry that I'll act on them I've had images of myself trying to hurt myself, typically by way of stabbing myself in the stomach. Despite these uncomfortable thoughts, all signs point to an inability to act on them:

I don't like pain I don't use drugs I have never planned anything

As well as instincts... when I am near a wall or hard surface, if I try to punch it: I stop. Unless I'm right up next to the wall, my fist literally stops.

Is this more of an OCD type issue? Should I be worried that if these thoughts keep happening I'll become fearless to them? I do take medication, and have dealt with anxiety for years, and depression on and off. It just gets annoying.
