Does anyone know or had any experience of anything off the shelf that we could adapt / integrate into an existing MS Stack (Azure DB and Winforms/ASP.NET MVC app) that could allow us to build, deploy and capture form data (for example urine output form, site audit report etc etc). We need to and want to integrate this form data direct to our entities (people) and capture /link data to it.

The forms will need to be mobile friendly and browser friendly. What we then need to do is integrate the data captured into our databases etc For example a carer clocks in and one of the tasks is to record liquid intake on a form. This form would be accessible from the app but would capture liquid intake for that client.... That data needs to appear somewhere in our system. I'm trying to shortcut a load of development which would be around the form building and publishing.
