I know I can handle these in code by having collection navigational properties at both ends.

I need to design an ER diagram before I even get to this stage though, so my question is, should I show the many to many relationship on the ER diagram or should I include the joining table that entity framework will create eg: Students - Courses - - StudentCourses.
